How to file an ADA complaint with Paraquad Inc.:

If you have a complaint about the accessibility of our transit system or service, or believe you have been discriminated against because of your disability, you can file a complaint using the ADA Complaint Procedures Form.

  2. In addition to the complaint process at Paraquad Inc., complaints may be filed directly with the Federal Transit Administration, Office of Civil Rights, Region 7, 901 Locust St., Suite 404, Kansas City, Mo. 64106 or call (816) 329-3920.

  3. Complaints must be filed within 180 days following the date of the alleged discriminatory occurrence and should contain as much detailed information about the alleged discrimination as possible.

  4. The form must be signed and dated, and include your contact information.

TITLE VI

Paraquad Inc. posts Title VI notices in public areas of our agency and on our buses and/or paratransit vehicles.

Paraquad Inc. operates its programs and services without regard to race, color or national origin, in accordance with Title VI of the Civil Rights Act of 1964.

    We collect your email address if you sign up to receive our updates. We also collect your name, mailing address, phone number, and email address if you choose to donate to Paraquad. Our partner, Blackbaud Inc., handles credit card processing for Paraquad.

We will not share your information with any third party outside of our organization, other than as necessary to fulfill your request. Unless you ask us not to, we may contact you via email in the future to tell you about special events, new products or services or changes to this privacy policy.
