Transitional Resources & Services

Paraquad can assist residents of nursing homes move into the community. An independent living specialist will work with each person to address relevant concerns, provide transitional resources and learn skills that will help them as they live independently.

Finding stable, affordable housing may be a challenge for someone with a disability. Our specialists identify and share living options in the St. Louis area. We factor in income, accessibility features and more to help you find the right option for you in our community. 

Finances

Managing finances may be difficult for any adult living on their own — but there are resources to help. Our specialists assist you in tackling obstacles to your financial health, whether that be employment barriers or public safety net constraints. Together, we pinpoint the best transitional resources to help you maintain your financial independence.

Personal care

People with disabilities may need assistance in day-to-day personal care activities. Our transition specialists assist you in defining your needs and hiring a personal care attendant to fulfill them. From personal hygiene to housekeeping, our resources empower you to live with greater independence in the community.

Independent living

Living independently involves multiple additional factors you may not consider without discussing with an independent living specialist. We discuss all areas of living in the community, including recreation, transportation, diet and beyond. 

Transition Timeline

The transition process of moving out of a nursing home into the community can take anywhere from six months to two years. While our specialists offer transitional resources and experience in the planning process, the person with a disability is in control and a successful move depends largely on his or her involvement.

People at risk of nursing home placement can receive referrals for resources, assistance and other information from Paraquad’s Information and Referral Department to help prevent institutionalization.
