Transition Services

Paraquad can help residents of nursing homes move into the community. An independent living specialist will work with each person to address relevant concerns and learn skills that will help them as they live independently.

People can work with their specialist to learn about:

• Housing

• Finances

• Personal care

• Financial resources

• Independent living

The transition process of moving out of a nursing home into the community can take anywhere from six months to two years.

While our specialists offer resources and experience in the planning process, the person with a disability is in control and a successful move depends largely on his or her involvement.

People at risk of nursing home placement can receive referrals for resources, assistance and other information from Paraquad’s Information and Referral Department to help prevent institutionalization.
